SUPPORT COORDINATION

Support coordination is a capacity building support which helps you to understand and use your NDIS plan to pursue your goals. It helps you to connect with NDIS providers, the community, mainstream and other government services and build your confidence and skills to use and coordinate your supports. A support coordinator delivers support coordination services.

Our support coordination service help you:

• Find your way through the NDIS process and assist you to identify your goals and select the best services to meet your needs.

• Assist you to monitor your NDIS plan and budget.

• Discuss accommodation needs, including in-home care, respite, short-term accommodation.

• Support NDIS plan reviews.
